Event Request

Public Health Madison & Dane County partners with community organizations to support events that promote health, connection, and access to resources. Use this form to request Public Health participation, outreach materials, or health education at your event.

Please submit your request at least 4 weeks in advance if possible so we can coordinate across programs. A public health staff member will follow up within 2–5 business days. Submitting this form does not guarantee participation. Availability depends on program capacity and alignment with community need.

Vaccines
Should vaccines also be available for people with insurance?

We generally provide COVID and flu vaccines for uninsured adults, and all other vaccines for uninsured children or children on BadgerCare. If you need vaccines for insured adults, our participation will depend on if we can coordinate with a pharmacy who can vaccinate insured people.

Have people you serve or partners shared interest in a vaccine clinic or easier access to vaccines?
If we’re able to attend, can you help promote the vaccine clinic within your community?

We can provide printable and digital flyers to support outreach.
