Healthy Communities Fund Request for Proposals

We are requesting proposals from organizations in Dane County whose work helps us achieve health equity. Proposals should focus on the impact that root causes of health have on health outcomes. This funding is open to new and existing projects and programs, including the operations of groups or agencies. Higher preference (scores) will be given to proposals for projects which:

  • Are supporting new or expanding efforts aimed at improving health equity
  • Clearly explain how these funds are critical to the outcome or success of your proposal
  • Benefit areas or populations impacted by systemic marginalization AND where the funded person or organization is currently in community with this group.
