School Vaccination Clinics

In partnership with SSM Health, GHC, UW-Health, and Madison Metropolitan School District, we're offering vaccines at clinics. 

  • The vaccines are free.
  • For school aged kids 5 and older.
  • Appointments are strongly recommended.

We can vaccinate you if you:

  • Do not have insurance
  • Are under 19 and are Alaskan Native or American Indian
  • Have BadgerCare
  • Have Quartz, GHC, or Dean insurance

  • May 11, 2024 (Saturday) from 10:00am to 2:00pm: Holtzman Learning Center (333 Holtzman Rd, Madison)
  • October 7, 2024 (Monday) from 12:00pm to 4:00pm: Mendota Elementary School (4002 School Rd, Madison)
  • October 14, 2024 (Monday) from 4:00pm to 7:00 pm: Leopold Elementary School (2602 Post Rd, Madison)
  • October 19, 2024 (Saturday) from 10:00am to 2:00pm: Holtzman Learning Center (333 Holtzman Rd, Madison)

What to Bring to the Clinic

  • Completed forms. Having these completed helps you get through the clinic faster. If you don't have a printer or can't fill it out ahead of time, we will have copies at the clinic. 
  • Your child's immunization record. Bring a print out of which vaccines your child has gotten. 
  • Your insurance card, if you have insurance. SSM Health will bill your insurance company for the cost of the vaccine only. There will be no charge to any person. All are welcome even if you do not have insurance.

Preparing for Your Visit

  • Illness or change of plans. Please stay home if you are sick. If you are not able to attend your appointment due to illness or other reason, that is fine. You do not need to let us know.
  • Arrival time. Please arrive no earlier than 5 minutes before your appointment time. This will help the clinic move faster and make parking easier.
  • What to wear. There is no privacy in the vaccine area, so we recommend you wear a short-sleeved shirt and bring a jacket or sweatshirt for warmth, as the temperature in the clinic can fluctuate. You'll be in the clinic about 30 minutes, so you may want to bring something to keep busy.
  • Read the Vaccine Information Statement or fact sheet for requested vaccine(s).
