• No ID needed

We can vaccinate:

• People six months and older who:
  • Have no insurance for immunizations
  • Have Medical Assistance or BadgerCare
  • Are under 19 and Native American or Alaska Native
  • Have most other types of insurance
  • If you have insurance, please try to bring your insurance information
