Whether you tested at a clinic, hospital, school, pharmacy, or used a home test, please take a few minutes to report in to Public Health. You can help us slow the spread of COVID-19 in Dane County and learn what you and your household should do next.

Complete the online interview, which will give you recommendations to help keep yourself and those around you safe. The questions will help you identify how long you need to isolate, who you might have been around while you were infectious, and what guidance you should share with your close contacts. It takes about 10 minutes to complete.

If you already tested positive at a testing site and provided a valid cell phone number to your healthcare provider or COVID Connect, you may receive a text from Public Health to fill out the COVID questionnaire. The text is sent from either 91972 or 94902. You can always call us at (608) 266-4821 to verify the authenticity of a text if you’re not sure. We will never ask for your social security number or any financial information.

We also strongly recommend following up your home test with a second test from a health care provider or at a community testing site to confirm your test result. It is especially important to get a confirmatory test if you need to prove later that you’ve already had COVID-19, such as for foreign travel or a medical procedure.
